Our on staff Licensed Social Worker and Registered Nurse partner together to make sure a comprehensive medical, social, and behavioral assessment explores all resources in individual care needs.
AHF Targeted Case Managers work with various community resources, DCFS, DMH, and Probation Department to bridge families with resources that will promote healthy family reunification. The goal is to supply the individual and families with tools and resources that will strengthen family bonds to thrive in their communities.
Intensive Care Coordination is a wide range of services that are provided by AHF which aims to provide continuous integrated care for youth with complex care needs involving a multi-care system between medical, behavioral, and social, and educational systems.
We thoroughly coordinate care between all multidisciplinary systems with the individual youth care needs in mind.
While a lot of our youth have experience a fair amount of adverse childhood experiences, it is our goal to aid them in those difficult times through Crisis Interventions. All staff are certified in Crisis Intervention Prevention and are adept to assist youth as feasible as possible.
Crisis Intervention is a short term, immediate response to a situation that poses a threat to an individuals physical, emotional or mental well-being. The aim is to stabilize the person in crisis, reduce the potential for harm, and to provided support and resources to help them cope and recover.
At AHF it is our commitment to aid our youth and Non-Minor Dependents in the best possible future through knowledge, vocational programs, and as a liaison to linkages to Career pathways. Non-minor Dependents are placed in specialty pathways and aided with obtaining: Identification/Driver License, Birth Certificate, Social Security Cards, Disability, Transitional housing, Financial Aid, Vocational/University Educational sources. We also aid in obtaining any vital information in family connection.
In our Initial Child Family Team meeting it is a priority to identify ALL necessary referrals to provide to both the adolescent/NMD and their families to support successful reunification or set up for independent success.
Referrals will include linkages with: Adoption, LBGTQ+, CSEC/SOGIE, Substance abuse, and Gang Prevention.
